Dr. Justin Slager

Dr. Justin Slager moved to Cookeville with his wife and two daughters in 2015 after serving as a dentist in the United States Air Force. Dr. Slager hails from Michigan but has quickly come to call Tennessee home and is actively involved in Young Life, Putnam County YMCA, The River Church, and Highland Rim Academy. His compassionate approach to dentistry will leave you feeling cared for and listened to.

Dr. Slager is always in constant pursuit of dental excellence. He regularly attends Dental CE each year allowing him to constantly add to his skill set.

Dr. Slager is now a Fellow of the Academy of General Dentistry! What does that mean for you and your family?

Earning Fellowship takes dedication to dental continuing education (CE) beyond graduation. Only about 6 percent of general dentists in the U.S. and Canada are AGD Fellows. When you see a dentist with who is a Fellow of the Academy of General Dentistry, you can rest assured that they care about knowing the latest techniques and best practices in dentistry. AGD Fellows practice these techniques in a classroom setting multiple times a year — not every dentist does this.

To become an AGD Fellow, a dentist must:

  • Complete over 500 credit hours of CE.
  • Earn 350 of those 500 credits in live courses.
  • Pass an exam equal in difficulty to board certification exams.

Jenny Miller

Dental Hygienist

Jenny graduated from dental hygiene school at St Petersburg College in 1994 and since then, she has loved everything that dentistry has to offer. Jenny and her husband (also her high school sweetheart) moved to Tennessee in 2007 from Florida. They love living the country life with their six cats and 2 dogs. Her favorite things to do are hang out at the lake, do crafts, Bible journaling, cook and bake.


Rachel Holman

Dental Hygienist

Rachel grew up in Livingston, TN and graduated from East Tennessee State University with her Bachelor’s in Dental Hygiene in 2017. She worked as a hygienist in Johnson City and Knoxville before making her way back home to the Upper Cumberland and is so glad to be home! She loves providing thorough care while putting patients at ease. Rachel and her husband, Grant, are expecting their first baby girl in April 2022. She enjoys traveling, going to concerts, watching football, reading, spending time with her family and loving on her golden retriever, Izzie.


Carolina Bailey

Dental Hygienist

Carolina grew up in Northern Illinois and graduated from Parkland College in 1999 earning her AAS and Dental Hygiene degrees. She later moved to Colorado, where she and her husband raised their daughter Ellarie in an outdoor lifestyle of hiking, biking, skiing and camping.

Carolina and her family are blessed to have found their new place to call “home” in Cookeville, TN. They enjoy taking their English Setter Sirly with them everywhere, including kayaking & hiking adventures!


Samantha Bos

Dental Assistant

Samantha Bos was born and raised in Putnam County. Growing up, her mother worked in dentistry and was so happy helping others that she is the reason Samantha works in dentistry today. Samantha graduated from Upperman High School in 1993 and immediately started working as a hygiene assistant with a local dentist. Samantha is married with three children and they live in Buffalo Valley. They love spending time together and with friends, going to basketball games and the lake and riding ATV’s and dirt bikes. She has worked at this office since 1994 and she loves every aspect of her job.


Katilin Walter

Dental Assistant

Katilin originated from Cookeville, TN but is a Sparta Warrior at heart. She is reliable, loyal and fun! She takes pride in making people laugh and loves to bake. She enjoys spending time with her children, Boston and Jaxxon. Katilin was raised by her Nana who she calls her hero and she also has a brother who is in the Navy and a sister from Arkansas. She strives to take the best care of her patients with gentle hands and to comfort them with some laughter. She has been loving her career in the dental field since 2013!


Christy Moore

Dental Assistant

Christy Moore is a Cookeville native, whose passion for dentistry began at a young age when she was given the opportunity to become a dental assistant at the age of 17. She made student of the year at Remington College Nashville’s dental assisting program and graduated with a 4.0. She has learned so much at Slager Family Dental Care and is excited to continue to learn and grow as a member of our team. In her free time, she enjoys spending time with her husband, Jonathan, her dog and cat, and her friends and family. She also loves camping, hiking, being outside, and singing.


Kim Weaver

Insurance & Billing Coordinator

Kim, a native of Johnson City, TN, and her family are thrilled to call Tennessee “home” again after spending the last 33 years in Louisiana. Kim is excited to continue her dental career as part of Slager Family Dental Care’s Admin Team after having served many years in a general dentistry practice in Baton Rouge. She is always happy to assist patients in understanding and utilizing their dental insurance benefits.

Her husband, Pat, a lifelong educator, is a chemistry teacher at Upperman High School and their youngest son, Grant, is a 3rd generation Golden Eagle at Tennessee Tech. Their family is part of the Stevens Street Baptist Church community and enjoys traveling and all outdoor activities, especially hiking, exploring, and enjoying all the beauty and charm of Tennessee they remembered from their childhood.


Stephanie Garland

Administrative Assistant

Stephanie and her family moved to Tennessee from Illinois in 2017. She and her husband Aaron have two daughters, Natalia and Charlotte. They love being outdoors and enjoy raising animals on their mini-farm.

Stephanie has been working in the dental field since 2007 and she really enjoys it. She is a people person and really loves connecting with patients and helping them with their dental care needs. One of the things she loves most about her job is how a positive experience at our office can change someone’s life.


Hannah Jaffe

Administrative Assistant

Hannah is a native of Cookeville, attending both Cookeville High School and Tennessee Tech. After graduating from Tech, she lived briefly in Australia and near New York City before returning home to the south. Hannah was a certified pharmacy technician and an audio technician, before joining our dental team.

She has a wonderful circle of close friends and loves spending time with them camping or being at the beach, but especially the beach!
